Employment – Community Health Worker

Ko’olauloa Site (Located in Kahuku Hospital)

Position Summary

A primary purpose of the Community Health Worker is to provide health care outreach services to Native Hawaiian individuals and families, to improve a client/patient’s quality of life. Community Health Worker carries out functions within culturally, appropriate service delivery programs in the community that include client identification; assists or provide health promotion/education activities. The Community Health Worker is a community member who serves as a “bridge” between health care consumers and providers to improve health and promote healthy lifestyles.

Essential Duties and Responsibilities

  • Explains programs and procedures and answers general questions to potential clients, with means of providing services to clients, inclusive of assessment/reassessment; coordination of provider services; and, the development of an individualized care/service plan with a prescribed time-frame.
  • Provides case management of clients/patients.
  • Maintains client records.
  • Utilizes tracking mechanism, database, created for the purpose of identifying, tracking and providing additional services as needed.
  • Attends and participates in community meetings and health fairs to understand community issues and build relationships with community members and agencies.
  • Performs other related duties and responsibilities as assigned by the Client Services Manager.

Qualification Requirements

  • Minimum Associates degree from an accredited university or college, preferably, in a Health or Human Services related field.
  • Must have minimum of 2 years general experience which demonstrated the ability to establish and maintain relationships with people; provide and elicit pertinent information; and, recognize and identify basic human needs and motivations
  • Access to vehicle and adequate no-fault auto insurance coverage, with a valid driver’s license and a clean driver’s abstract.
  • Valid tuberculosis clearance within 1 year.
  • Computer literate.

Working Hours

8:00 a.m. to 5:00 p.m., Monday through Friday. Some evening and weekend work may be required.
