Position Summary
The Community Health Nurse provides generalized Community Health Nursing services to Native Hawaiians residing on the island of O’ahu. Primary responsibilities include a focus on case management and health education within a cultural framework. The Community Health Nurse provides services in a variety of venues within the community, working with individuals, families, and groups with the purpose of empowering, educating, and promoting the health and well-being of our Native Hawaiian community.
The incumbent designs and implements health education programs to increase awareness and knowledge for the improvement of health in target communities. Promote, maintain, and improve individual and community by assisting individuals and communities to adopt healthy behaviors. Collect and analyze data to identify community needs prior to planning, implementing, monitoring, and evaluating programs designed to encourage healthy lifestyles, policies, and environments. Keeps up-to-date on changes in health care technology to keep health education program material current. Serves as a resource to assist individuals, families, other professionals, or community/agencies in public health trends and issues.
The Community Health Nurse will develop and manage nursing care plans, instruct patients and their families in health care; and assist client and or family members to become proficient in self care/self management. Develop and sustain community partnership plan and implement programs. Work with community leaders, community, public and private agencies, teachers, parents, physicians, and the community in general to provide appropriate health care services.
Essential Duties and Responsibilities
1. Health Education Programs
Designs and implements health education programs/curriculum to increase awareness and knowledge for the improvement of health in target communities.
Promote, maintain, and improve individual and community health by assisting individuals and communities to adopt health behaviors.
Project planning/evaluation, training, condition building, community networking and community health education.
General
Fulfills program reporting requirements. Responsible for monitoring progress towards funder’s objectives.
Formulates and communicates to internal and external constituencies the mission, objective and service priorities of the projects assigned.
2. Case Management
Provides case management services as needed based on the client’s need and/or case management team.
Conducts home assessments as needed with the Client Services Department.
Identifies clients who require skilled clinical intervention or assistance. Provides appropriate referral for clients.
Conducts follow-up interviews with clients and health care providers.
General
Performs clinical activities as required for assessments and screenings.
Promote, maintain and improve individual and community health by assisting individuals and communities to adopt healthy behaviors.
Collect and analyze data to inform program design prior to implementation.
Collect and analyze data during program implementation.
Evaluates program effectiveness and service delivery.
Contributes to monthly, quarterly, and annual reports.
Maintain professional competency utilizing a variety of venues.
Other Duties and Responsibilities
1. Participates in monthly staff meetings.
2. Performs other duties as required or assigned by supervisor to further the mission of the organization.
Working Conditions
Indoors and outdoors, under a variety of conditions in the community.
Working Hours
Normally 8:00 a.m. to 5:00 p.m., Monday through Friday. Some evening and weekend work required at the direction of the supervisor.
Equipment Use
Computer, copier, telephone, fax machine, and other standard office equipment; stethoscope, sphygmomanometer, cholestech (lipid) DCA 2000 (HgbA1C), electronic vital signs machine, scale (weight) stadiometer (height), glucometer, syringe, and various solutions for possible immunizations/vaccinations, and other medical equipment as needed; personal vehicle or 7 passenger vehicle.
Mental Demands
Duties require observing and reporting client health care needs and services; dealing continuously and tactfully with clients, service providers, coworkers, and Native Hawaiian Community; working under occasional pressure and deadlines; following instruction in company policies and procedures; complying with KOM Code of Ethics.
Communication Demands
Maintains level of interaction, which includes the ability to work and interact at a professional level with diverse organizations and audiences (health professionals and lay people). Requires strong verbal and written communication, which entails preparation of oral and written reports, memos, and other communications in a clear, concise, and professional manner; taking accurate phone messages; communicate the written and spoken work with tact, diplomacy, and/or authority when necessary.

The employee must occasionally lift and/or move up to 20 pounds. Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus. Qualification Requirements
Knowledge, Skills and Abilities Requirements
Demonstrated knowledge of Native Hawaiian cultural values and practices in the delivery of health care services.
Knowledge of community, health and human service agency resources.
Knowledge of community needs.
Knowledge of technology and automation.
Knowledge of principles and practices of effective case management.
Working knowledge of software applications used by KOM: eClinical Works, MANA.
Skilled in nursing fundamentals (i.e. lab draws, vital signs, health history, etc.).
Excellent problem solving, organizational, detail oriented and time management skills.
Ability to integrate Native Hawaiian cultural sensitivity methods in delivery of service.
Ability to Advocate on behalf of the client by means of locating resources to meet as many of the client’s needs as possible and connecting the client to identified resources; to assess the client’s progress and the effectiveness of the treatment plan interventions; to ensure the client receives services in a timely and effective manner.
Ability to work with all service providers to ensure service integration.
Ability to document interactions with clients and service providers, to facilitate communication, case review, and decision making.
Ability to address the public and professional groups.
Ability to prepare and maintain reports and records.
Ability to work effectively and independently with various levels of Clinical and Administrative managers, coworkers and staff within KOM and with community leaders and provider/agency representatives.
Ability to establish and maintain effective working relationships with employees, other agencies, and the public.
Ability to maintain composure under time restraints.
Ability to maintain accountability and confidentiality.
Ability to prioritize tasks and demonstrate high integrity.
Other Requirements
Have a valid driver’s license and use of a personal vehicle for work with proof of adequate automobile insurance coverage. May use company vehicle for business of the organization.
Driver’s abstract without any moving violations within the past 3 years
Valid tuberculosis clearance within one (1) year.
Valid COVID Vaccination card at the time of application
Current CPR and First Aid for Healthcare Providers certification.
Education and Experience Requirements
Bachelor of Science in Nursing degree from an accredited college or university.
Current Hawaii State Nursing Licensure.
Prefer Master’s Degree in Nursing from an accredited college or university.
Prefer two (2) years of experience in patient care or community setting, program planning/evaluation, training, coalition building, community networking, and community health education.
Any combination of education and experience that would provide the necessary knowledge, skills and abilities to perform the essential functions of this position.
